b) A reference or testimonial (or a certified copy) from the headmaster of the last school you attended addressed to the Registrar at the
above address.
c) A Certified copy of your Passport/ID/ Voters card / a confirmation letter from your area chief.
This form must be accompanied by a non-refundable Application Fee of M280.00. The closing date for this application is 2
