Kindly be reminded that the forms for June/July 2021 intake are still available, and be aware that KIHSR does not transact in cash with application forms. Therefore payments of the forms should be deposited in a given Indo Zambia Bank account and should not be paid through mobile money or in person to any individual.

Application Requirements
Filled in Application Form.
Bank deposit slip.
Certified copies of Grade 12 results.
Copy of National Registration Card.
Two (2) Passport-sized photos.
All prospective applicants who meet the above requirements should:
A. Obtain Application Forms in person from Levy Mwanawasa Medical University – Admissions Office or the website: www.moh.gov.zm or www.cchs.ac.zm
B. Deposit a non-refundable fee of K200.00 (Kwacha Two Hundred only) into the Levy Mwanawasa Medical University Third Party Account No. 110110359429026 INVEST TRUST BANK ODYS BRANCH at any INVEST TRUST Branch countrywide.
C. All completed application forms should be accompanied with certified copies of certificates or statements of results; an authentic bank deposit slip; receipt from the Account’s Office and should reach the Academic Office by 31st May, 2021.
The above mentioned can be sent by email, posted or dropped off at the Admission offices at any of the two campuses. If your application has been successful, the Admission Office will contact you.

Application forms can be obtained from the Admission offices at the campuses. Lusaka applicants can also purchase the application form from our Lusaka office which is at Lotti House, 6th Floor, Room 28.
